Since 1920, Mount Carmel Guild of Trenton, New Jersey has supported the community of greater Mercer County. The Guild celebrated our Centennial Anniversary in 2020 during the COVID-19 Pandemic. Currently, the Community Support Food Pantry Program serves over 999 ~ 1,400 households each month. Presently, our Registered Nurses offer care to seniors enrolled in our Home Health Nursing Program every one to two weeks. Our work environment includes:

```Duties:```

- Provide skilled nursing care to patients in their homes

- Conduct comprehensive assessments of patients' health status

- Develop and implement individualized care plans

- Monitor and evaluate patients' progress and make necessary adjustments to the care plan

- Educate patients and their families on managing their health conditions

- Collaborate with other healthcare professionals to coordinate patient care

- Document all patient interactions and interventions accurately and timely

```Qualifications:```

- Valid Registered Nurse (RN) license in the state of New Jersey

- Two years home health nursing experience

- Excellent communication and interpersonal skills

- Compassionate, patient-centered approach to care

Pay: $33.00 - $34.00 per hour

Expected hours: 5 – 9 per week
